Is my profession regulated or not?

This question determines everything that follows. For a regulated profession, practicing the profession without recognition is prohibited by law—no one may work as a doctor or pharmacist without a license. For a non-regulated profession, you may begin working immediately; recognition is voluntary and serves only as proof for employers.

Special case for lawyers from non-EU countries: There is no access to the eligibility exam under the EuRAG. In practice, a new degree program leading to a state examination is required. Anyone with a law degree from a non-EU country should plan this path early on or explore a career alternative outside of the bar.

It is important to note a limitation that is rarely mentioned: The OECD notes that German employers often do not accept foreign qualifications even in unregulated professions. Regulation is therefore not the only hurdle. Even those in IT or business administration who do not need formal recognition still face a job market that values foreign degrees less highly.

What is the ZAB credential evaluation—and do I even need it?

The certificate evaluation from the Central Office for Foreign Education (ZAB) is an official document that classifies your foreign college degree and compares it to the German system. It costs 208 euros; a duplicate costs 104 euros. If you cancel, up to 50 percent may be retained.

An important note regarding the duration of the credential evaluation: The law stipulates three months—but the ZAB itself points out processing bottlenecks. Therefore, do not plan on three months as a reliable estimate, but rather on “three months as stipulated by law, though in practice it currently takes longer.” The high volume of applications is well documented: In 2024, 118,400 applications for certificate evaluation were received, 55 percent more than the previous year.

What language proficiency level does my profession require?

The requirements range from “no formal level required” to C2—and they vary significantly by federal state. Physicians need a B2 level in general language proficiency plus a C1 level in specialized language proficiency through the specialized language exam. Psychotherapists and teachers must demonstrate C2 proficiency.

Two state-specific considerations are important for planning: Saarland accepts the telc “German B2·C1 Medicine” exam without requiring separate proof of B2 proficiency. Hamburg and Hesse differentiate between qualifications from the European Economic Area and those from third countries. Moving to another federal state can therefore trigger additional requirements in the middle of the process.

The third part is the most surprising. It’s not enough to master technical terminology—you have to be able to switch between technical and lay language. This very skill of translation was never practiced during your studies because it came naturally in your native language back home.

How many fail—and why?

About one-third of doctors fail the medical terminology exam. In Baden-Württemberg, the failure rate was around 35 percent in both 2020 and 2021; in Saxony-Anhalt, it was 32.8 percent in 2024 and 30 percent in 2025. The figures have remained remarkably stable across years and federal states.

The comparison with the technical examination is what makes the findings meaningful. In the general knowledge examination for human medicine, approximately 84 percent pass by the third attempt, and 71 percent on the first attempt. What these two tables together mean: About one-third of medical students fail an exam that explicitly does not assess specialized knowledge. About 84 percent pass the exam that actually tests medical knowledge. So anyone who invests time and money in preparation is investing it in the wrong direction if they pore over textbooks instead of practicing language skills.

One figure that frequently comes up in discussions has been deliberately omitted here: The often-cited Bavarian rate of 52 percent dates back to 2017, the first year of the exam, and is not reliable. What can be substantiated is “about one-third.”

What is the knowledge exam, and how does it differ from the technical language exam?

The knowledge exam tests your subject knowledge, while the medical language exam tests your language skills. The knowledge exam is oral and practical, takes place in one day, lasts 60 to 90 minutes, and focuses on internal medicine and surgery. Candidates are allowed two retakes.

Appointments are scheduled one to six months after registration. It’s impossible to say with any certainty how long preparation takes on average—there are no official statistics on this. The only documented information is the timeframe for scheduling appointments.

The interim solution that many are unaware of: Section 10 of the Federal Medical Code allows for a temporary, supervised license to practice, typically for up to two years. This is often granted as soon as the candidate has obtained a B2 level and passed the specialized language exam—that is, before the knowledge exam has been taken. This allows the candidate to start working sooner and earn money while the process continues.

How much does all this cost?

For the medical certification process, the costs for the evaluation of academic records, the specialized language exam, and the knowledge exam quickly add up to a four-digit amount. The evaluation of credentials costs 208 euros, the specialized language exam costs between 420 and 595 euros depending on the medical association, and the knowledge exam costs between 980 and 1,200 euros.

In addition, there are costs for translations, certifications, and obtaining documents from the country of origin. These incidental costs are often underestimated because they consist of many small amounts.

What financial assistance is available—and what is not covered?

For academic healthcare professions, there is a dedicated, highly suitable course: the BAMF Special Course for Academic Healthcare Professions at Level C1, comprising approximately 600 instructional units. It is designed for students in medicine, dentistry, and pharmacy undergoing the recognition process starting at Level B2—and the BAMF covers the fee for the specialized language exam.

For procedural costs, there is the federal recognition grant: procedural costs ranging from 100 to 600 euros, certificate evaluation and qualification analysis up to 1,200 euros, and qualification measures up to 3,000 euros. Since January 1, 2025, the income limit has been 32,000 euros for individuals and 50,000 euros for joint tax filers. The program runs through 2027; enrollment is possible until June 30, 2027, and reimbursement is available until September 30, 2028.

Three things that are not eligible for funding—and a timing issue. The recognition grant does not cover language courses or language exam fees. Funding is not available for vocational language courses taken from abroad—that is, from a third country prior to entry into Germany. And there is no BAMF special course for legal professions. The timing issue: The funding application must be submitted before the recognition application—retroactive applications are not accepted.

Other options include the education voucher, which, however, applies only to AZAV-certified courses; the free, income-independent recognition counseling offered through the IQ funding program; the Jobcenter; and, in Hamburg, a dedicated scholarship program.

How long does it take?

For a third-country degree with additional requirements, expect 12 to 24 months from the application to the certificate. The statutory deadline for the initial decision is four months, but it does not begin until all documents have been submitted. According to the BIBB (data from 2017 to 2020), the process takes an average of about 15 months until the final decision is issued when compensatory measures are required.

In addition, there are waiting periods at each stage of the process: The ZAB’s evaluation of the diploma is scheduled to take three months, but according to the agency’s own information, it currently takes longer. Appointments for the knowledge exam are scheduled one to six months after registration. IAB Research Report 22/2024 shows that in half of the cases with additional requirements, up to 426 days elapsed between the notice of deficiencies and the final decision alone.

These months represent the exact time window for language preparation. Those who begin targeted professional language training upon submitting their application usually have a year or more until the professional language exam—and thus enter the exam, which otherwise causes about one-third of candidates to fail, with a significant head start.
